A mix-up between crew and a pilot caused the near grounding of a Greek capesize off Newcastle last year.

The Australia Transport Safety Bureau (ATSB) said the fully-laden 182,000-dwt Carras Hellas capesize Aquadiva (built 2010) was departing the port on 12 February last year when it passed through a section of the harbour channel known as the Horse Shoe.

Insufficient rudder was applied in time to effectively turn the ship, causing it to slew toward the southern edge of the channel, going over the limits of the marked navigation channel.
