The2,500-dwt Hyak (built 1967) hit a 27-foot vessel near Shaw Island in September, causing Patricia Whaley anda second mate to be suspended.
TheKitsap Sun reported an investigation found the collision was due to human errorand Whaley was fired as a result.
Thesecond mate has resigned.
The man on board the boat was injured, but is making a recovery.
