Victims' families packed the court in Gwangju on Tuesday, one holding up a sign that read: "You are not human. You are beneath animals."
A scuffle broke out between relatives and court security guards who tried to take the sign away.
The ferry sank in April, killing more than 300 people, many of them schoolchildren.
The captain, Lee Joon Seok, and three senior officers face homicide charges.
