Safety failings in mooring operations havebeen highlighted by UK accident investigators after a seafarer was pulledoverboard from a cargoship.

Sergey Gaponov, 40, from Russia, died after being dragged from the 3,700–dwt Sea Melody (built 1994) in the River Trent when his foot became trapped in a loop of rope last December.

The crewman was heard to call out for helpand his colleagues turned towards him and saw that his left leg had becomecaught.

They
