Japanese shipmanager Hachiuma Steamship has hit back at suggestions that there was an attempted cover-up over the death of a superintendent on the 105,000-dwt bulker Sage Sagittarius (built 2000), the third man to die on the ship over a six-week period.
The man, reportedly put on the ship to improve safety after two earlier fatal accidents, was crushed by a conveyor belt as the vessel was unloading in Japan on 6 October.
