A startling navigational blunder led the 75,000-dwt bulker Oliva (built 2009) to plough into Nightingale Island, Tristan Da Cunha, causing an environmental disaster in the middle of the Atlantic Ocean, according to an investigation by the ship’s flag state, Malta.

The report has been filed with the International Maritime Organisation (IMO), however, Transport Malta did not make a public release.

But its findings are now bound to add to the ongoing debate over seafaring standards and the human element in safe navigation.
