The reliability of the voyage data recorder (VDR), which is now mandatory, has again emerged as a significant problem in a major casualty investigation.
The Federal Bureau for Maritime Casualty Investigation (BSU) says it was unable to secure any data from the VDR onboard the MSC Flaminia.
As has happened in previous casualty investigations, the crew forgot to back up the VDR data in the chaos that surrounded the fire and abandon-ship operation.
