The man died on 15 July 2015 after falling from a ladder while carrying out pipe-cleaning work as the ship headed towards the Dutch port of Rotterdam.
A preliminary report from Germany’s Federal Bureau of Maritime Casualty Investigation (BSU) said: “No precautions were taken against a fall from the ladder with respect to personal safety [safety guard at the ladder] and personal protective equipment [fall arrest system].”
