Tiredness contributed to a tanker grounding in Canada as the helmsman appeared to misapply his captain’s instructions, an accident report has revealed.

The 9,200-dwt Nanny (built 1993) hit the bottom in Nunavut in October 2014, damaging its ballast tanks and causing water ingress.

The Transportation Safety Board of Canada (TSB) said fatigue contributed to helm orders being incorrectly applied.
