A "blame culture" is exacerbating the problem of seafarer deaths in enclosed spaces, according to shipmanagement association InterManager.
Dangerous time frames are being imposed for hazardous tasks and safety improvements do not happen because investigations encourage this culture, it said.
The association has carried out a survey of operators, managers and crew from almost 250 ships over the last three months to find out more about what they think on the issue.
