A Svitzer crewman was not wearing protective gear when he fell to his death from a tug in the UK, an investigation has found.
Engineer Kevin Jackman was crushed as he moved from the 267-gt Svitzer Moira (built 1998) to an unmanned vessel at Bristol on 29 December last year, the Marine Accident Investigation Branch (MAIB) said.
