Team work is vital during pilot operations if accident claims are to be avoided, according to a new report.

The UK P&I Club and Solis Marine Consultants launched a joint study that found human error and the breakdown of communications on the bridge is a major contributing factor in claims.

It said the bridge team must ensure that they know the transit to and from the berth, including the route agreed, details of the tugs to be used and berth information such as limitations on vessel size, turning areas, mooring plans and speed limits.
